Aldersgate At Home Services is a private pay organization. But often, long-term care insurance can cover our services.
Aldersgate At Home operates seven days a week. Hours vary depending on your individual needs. There’s a consecutive four-hour minimum for clients not residing on the Aldersgate campus, and we can also provide coverage 24 hours a day if needed.
For full-time caregivers looking for respite care to get out for a day, take a vacation or have surgery, it’s perfect.

Now off-campus clients can access the same high-quality supportive care from Certified Nursing Aides that Aldersgate residents enjoy.
Aldersgate At Home recognizes the emotional, physical, and mental challenges of caregiving. When a spouse, family member or friend provides daily care, it can take a toll. Adding a respite care break once a week, even for as little as four hours, can make a huge difference.
We’ll design a respite care plan that works for you and your loved one. Longer breaks—a full weekend or up to several weeks—can accommodate family trips or surgery. From meeting friends or family for lunch, attending a class or support group, having surgery or visiting the doctor, breaks can prove vital to a caregiver’s long-term well-being.

Those in the process of passing can do so comfortably—surrounded by family and friends—in their own home. We help clients and their families during these difficult times with one-on-one, 24-hour care from a Certified Nursing Aide. Plus, we partner with hospice and palliative care organizations to coordinate the best plan for a comfortable transition that honors your loved one’s wishes.
The last thing anyone wants is a new person in their home who doesn’t understand them. Our onboarding process for Aldersgate At Home health services is extensive so we can satisfy your needs. We get to know our clients’ personal preferences—what they love and any pet peeves. Our Client Care Coordinator will visit with you via telephone, web meeting and a home visit to get started and discuss whether you’re interested in health services, personal assistance or both.
If you’re interested in Aldersgate At Home services, the process usually looks like this:
Our Client Care Coordinator will help you complete some intake forms. This can be done with the client or their advocate such as a family member or other loved one. We can also help navigate long term care insurance, if needed.
A Registered Nurse will meet with you about the client’s needs and wishes and will prepare a custom care plan for your approval.
Our Scheduling Coordinator will determine which caregiver would be the best match for you/your loved one by considering personality, care needs, scheduling preferences, etc.
We start! Our trained team of In-Home Nurse Aides will help you/your loved one maintain a maximum level of independence by providing everyday, quality care.
Aldersgate At Home communications don't end with onboarding. We communicate regularly to ensure you are happy with our service. If an advocate is involved, we'll regularly report how your loved one is doing.
